- 주제: Privatizing Long-Term Care: Impacts of Medicaid Managed Long-Term Services and Supports on Nursing Home Quality and Resident Composition (with Michael Lepore, PhD; Alison Trinkoff, ScD)
- Abstract: With the aging of population, the demand for long-term care services and supports (LTSS) has grown and is expected to increase further. In response, Medicaid, the nation’s primary payer of LTSS, has increasingly turned to managed long-term care services and supports (MLTSS) programs, adopting managed care delivery systems in place of fee-for-service models. Exploiting the gradual implementation of the program between 2006 and 2014 in eleven states, we used a difference-in-differences approach to examine changes in nursing home quality of care and resident composition in states that implemented MLTSS compared to those that did not. Following program implementation, quality improved in terms of staffing hours per resident day. However, quality worsened in terms of proportion of long-stay residents experiencing a decline in activities of daily living and rate of hospitalizations. We also observe a significant reduction in the proportion of residents with Medicaid as their primary payer, and a decrease in the average level of care required by residents. These results emphasize the need for a more comprehensive evaluation of the costs and benefits associated with privatizing long-term care.
